Position: Homeless Prevention Case Manager (ESG)

Position Summary: Through our supported housing department, TGCW provides housing with case management, housing retention, homeless outreach, and other support services to those referred through Westchester County Single Point of Access (SPOA). We work so those with histories of homelessness and mental illness, or substance use disorders can live independently. We locate permanent housing for participants throughout Westchester County and make rent subsidies available through various government-funded programs administered by: HUD City of Mount Vernon OMH Westchester County DSS OTDA Westchester County DCMH OASAS
Reporting to the Director of Housing, the Homeless Prevention Case Manager will work with participants to maintain independence in their community by teaching life skills (i.e., budgeting, bill payment, conflict resolution, etc.) and helping individuals find available housing, secure housing subsidies and other entitlements, and access community resources while embracing a harm reduction-based approach. This role will also focus on the specific interpersonal issues and problem behaviors that threaten housing retention and provide counseling and support to resolve them.
Responsibilities: Support a high-performing culture aligned with TGCW core values and B.A.S.I.C Tenets: Balance, Accountability, Satisfaction, Inclusion, and Communication. Responsible for identifying young adults between the ages of 18 and 26 in Mount Vernon in need of housing. Coordinate with the Homeless Liaison in Mt. Vernon School District to identify eligible families to identify families that can live independently financially with a little bit of support. Work with families and young adults to provide links to employment to ensure families are financially stable so they can maintain housing. Help participants understand their leases and tenant/landlord responsibilities, arrange utilities, obtain furniture, make moving arrangements, and develop sustainable household budgets.
Assess participants' needs and provide referrals, linkages, and wrap-around services as needed to substance use treatment, entitlement benefits, housing, and appropriate community resources. Conduct appropriate follow-up with participants and/or service providers as needed. Responsible for encouraging participants to seek help from local support services and advise them on how to minimize risks to their health and losing their housing. Maintain complete records, daily activity logs, progress notes, mileage logs, and other reports as required. Maintain confidential information within guidelines established by HIPPA and TGCW's Confidentiality Policy. Participate in professional development activities such as trainings, webinars, and continuing education services. Complete HUD Timesheet.
Qualifications: BA/BS in Social Work or related field (Human Services, Psychology, etc.) Valid NYS Driver's License with a clean driving record with the ability to maintain both in accordance with TGCW insurance guidelines. 2+ years of experience working within the Mental Health and/or Substance Use field with a diverse population. Knowledge of Mt. Vernon, Westchester County, and availability of community and governmental services and resources. Knowledge of local tenant and rental housing law. Ability to take the initiative, prioritize, and meet deadlines independently. Have adequate writing skills to complete reports, case notes, and other professional documentation quickly and organized manner. Knowledge of computer database operation and have basic data entry skills.
