Position: Nurse, Access Center
Duties and Responsibilities: The clinical team for The Guidance Center of Westchester’s Certified Community Behavioral Health Center (CCBHC) Access Center is responsible for providing quality coordinated crisis response, screening, engagement and linkages to care for mental health, alcoholism/addiction and other substance use, physical health and supportive services. The CCBHC Access Center interdisciplinary team includes psychotherapists, psychiatry, medical/nursing, care managers, peers, an engagement specialist, a court liaison and 24/7 Intake/Hotline staff. With the support of federal SAMHSA CCBHC funding, The Guidance Center of Westchester is able to provide services to individuals of all ages regardless of ability to pay, insurance or residence. The Access Center nurse is responsible for providing medical support and monitoring to TGCW’s CCBHC program participants.
Provides medical assessments and care, health education, injectable medications and medication/wellness education. Screen and monitor clients for medical problems and side effects. With client’s input and consent, develop strategies to maximize the taking of medications as prescribed. Engage in health promotion, prevention and education activities (i.e. assess risky behaviors and attempt behavior change). Communicate and coordinate services with other medical providers. Administer and document medication treatment in EMR. Participates in case consultations and conferences, as well as crisis support as needed in office or in the community via mobile crisis vehicle as needed. Manages physical health integration initiatives. Participates in interdisciplinary team meetings and trainings.
Qualifications: Registered Nurse. ANCC Certification Bilingual (Spanish/English) preferred.
2+ years’ experience providing care within behavioral health program for the full age range, including crisis care, high-risk and addiction populations. Familiarity with Westchester County System of Care and resources for all age groups and the full range of behavioral health and medical needs.
Skills / Knowledge Required: Expertise in medical, mental health and addiction crisis needs of children, teens, adults and elderly. Exceptional interpersonal, engagement, crisis stabilization, communication and educational skills. Familiarity of physical health integration models of care. Ability to provide nurse services in-office and in the community for crisis stabilization as needed. Travel Requirements: 25-50% throughout Westchester as needed to support mobile crisis for TGCW’s CCBHC.
Hours: Part-time, 14-19 hours per week
Salary: Commensurate with experience
