Position: Peer, Access Center
Duties and Responsibilities: The clinical team for The Guidance Center of Westchester’s Certified Community Behavioral Health Center (CCBHC) Access Center is responsible for providing quality coordinated crisis response, screening, engagement and linkages to care for mental health, alcoholism/addiction and other substance use, physical health and supportive services. The CCBHC Access Center interdisciplinary team includes psychotherapists, psychiatry, medical/nursing, care managers, peers, an engagement specialist, a court liaison and 24/7 Intake/Hotline staff. With the support of federal SAMHSA CCBHC funding, The Guidance Center of Westchester is able to provide services to individuals of all ages regardless of ability to pay, insurance or residence. The peer position teaches effective wellness self-management skills through the lens of lived experience. Provides clients and example of what they can strive for in their recovery. Builds relationships with clients leading to the development of life goals and enhanced engagement in recovery. Links clients to resources, services and natural supports internally at TGCW and in the external community to support attainment of recovery goals and reduction of isolation. Works with clients to increase positive social connections and develop stronger family connections, improve life skills including obtaining necessary resources and empower the client to increase wellness self-management. Documents all interactions in EMR. Promotes linkage to services by assisting clients with the coordination of appointments, providing appointment reminders and escorting and/or driving to appointments based upon the client’s needs and preferences. Participate in program staff/case conference meetings to promote efficient communication and care. Assists with supervised toxicology screens. Participates in team responses to crisis calls in the community. Assists with record keeping and documentation for SAMHSA CCBHC funding. Participates in interdisciplinary team meetings.
Qualifications: Having personal experience relevant to services provided Skills / Knowledge Required: High School Diploma or GED Demonstrated peer support or mental health experience Great communication skills Travel Requirements: 25-50% throughout Westchester as needed.
Hours: Part-time, up to 19 hours per week.
Salary: Commensurate with experience
