Psychosocial Rehabilitation Specialist


Position Summary:

Psychosocial Rehabilitation (PSR) services are designed to restore, rehabilitate, and support a child’s/youth’s developmentally appropriate functioning as necessary for the integration of the child/youth as an active and productive member of their family and community with the goal of achieving minimal on-going professional intervention. Services assist with implementing interventions on a treatment plan to compensate for, or eliminate, functional deficits and interpersonal and/or behavioral health barriers associated with a child/youth’s behavioral health needs. Activities are “hands on” and task oriented, intended to achieve the identified goals or objectives as set forth in the child/ youth’s individualized treatment plan.


Responsibilities:

• Assist individuals in developing personal and community competence using rehabilitation interventions and individualized, collaborative, hands-on training to build developmentally appropriate skills;

• Support individuals to improve self-management of the negative effects of psychiatric, emotional, physical health, developmental, or substance use symptoms that interfere with a person’s daily living;

• Use collaboration, partnerships and mutual supports to strengthen the child’s community integration in areas of personal interests as well as other domains of community life including home, work and school;

• Delivery of services in individual and/or group modality;

• Services are delivered in a trauma informed, culturally and linguistically competent manner;

• Coordinate with other providers involved in the individual’s treatment;

• Timely and accurate documentation of contacts;

• Attend required supervision, meetings and trainings;

• Report incidences and issues to supervisor immediately;

• Ensure delivery of services as outlined by service plan;

• Continuous monitoring that family choice and voice is present in service delivery.


General responsibilities:

• Attends workshops, conferences and staff development training seminars on current practices as directed by the Chief Executive Officer or Program

Director to acquire continuing education in the appropriate field; implements what has been learned;

• Maintains cooperative relationships with referent and community resources

• Undertakes related duties as assigned;


Administrative Relationships:

The Psychosocial Rehabilitation Specialist reports directly to the Program Director or designee and collaborates with other staff.


Qualifications:

At a minimum, staff must be 18 years old and have a high school diploma, high school equivalency preferred, or a State Education Commencement Credential with a minimum of three years’ experience in children’s mental health, or a Bachelor’s degree in social work, psychology, or in related human services; or a Master’s degree in social work, psychology, or in related human services. Must have basic computer literacy and excellent verbal and written skills. Valid driver’s license required.


Reasoning Ability:

Ability to apply common sense and understanding in order to carry out instructions furnished in written, oral or diagram form. Ability to deal with problem solving several concrete variables in standardized situations.


Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; climb two step ladder, use hands to handle or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 5 pounds and occasionally lift and/or move up to 10 pounds.
