Hudson Valley, NY April 11, 2024—The Hudson Valley Care Coalition (HVCC), a recently formed
alliance between Hudson Valley Care (HVC) and Coordinated Behavioral Health Services (CBHS),
expands its collaboration with Unite Us, the nation's trusted technology partner for social care
transformation, to leverage its closed-loop referral technology and coordinated care network across
more than 50 agencies the Hudson Valley region.
Unite Us will serve as the foundational technology infrastructure for the HVCC network, which
includes Federally Qualified Health Centers (FQHCs), health plans, behavioral health services,
substance use providers, hospitals, I/DD providers, and social care needs community-based
organizations (CBOs), to foster a sustainable ecosystem of health and social care across the region
in preparation for New York’s 1115 Medicaid Waiver demonstration.

Last September, CBHS adopted Unite Us’ closed-loop referral technology to send and receive
secure, electronic referrals and measure outcomes in order to address people’s health-related social
needs (HRSN) and Unite Us has since onboarded more than 180 community programs in the
Hudson Valley. The new alliance has empowered HVCC to build o n this momentum by expanding
the number of accountable organizations in the network in order to enhance the accessibility and
impact of these critical resources, such as housing, transportation, and mental health services.

effective support, ultimately facilitating the journey towards rehabilitation and recovery for those
we serve."
